GENERAL DESCRIPTION OF DUTIES AND RESPONSIBILITIES:
Camino Real Community Services, in partnership with an independent school district, provides trauma-informed mental health care to children and adolescents, addressing severe emotional and behavioral challenges, including co-occurring substance use issues. The Child & Adolescent Case Manager delivers tailored interventions such as screenings, case management, individual and family rehabilitative sills training, and crisis response. Services are integrated into the school setting through a structured module that includes on campus presence, early identification of at-risk students, and collaboration with teachers and counselors to create individualized support plans. The Case Manager also provides training for school staff on trauma-informed practices and serves as a critical resource for de-escalation and safety planning during crises. The case manager extends services to homes, agency offices, and community locations to ensure continuity of care. Acting as a liaison between families, the school district, and local agencies, the case manager facilitates access to additional resources, including psychiatric and substance use treatment. This role requires flexibility, extensive travel, and strong collaboration with community partners to deliver high-quality mental health support. By integrating services into schools and communities, the case manager helps foster resilience and create healthier environments for students and their families. These service interventions may be provided at home, in office, and/or in community settings. May assist the director in working relationships with local and state agencies and resources. Interagency collaboration is essential to this position. The job requires extensive travel for community-based service provision and flexible hours and is scheduled to meet the demands of the clients served.

MINIMUM JOB QUALIFICATIONS:
Applicants must meet the following minimum qualifications to be eligible for the position:
- Bachelor’s Degree from an accredited college or university, with a major in (or the equivalent to a major, i.e., 24 hours of course work) in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, education psychology, early childhood education, or early childhood intervention; OR a registered nurse. Must meet QMHP qualifications.
- You must possess good computer skills.
- You must be able to use a personal vehicle for travel to provide job duties.
PREFERRED JOB QUALIFICATIONS:
- Prior working experience with mental health.
- One (1) year of experience working with at risk youth.
- Prior working experience as a case manager.
- English/Spanish skills; demonstrated ability to read, write, and speak English and Spanish.
- Local resident of Atascosa County (Jourdanton, Poteet, Pleasanton).
KNOWLEDGE, SKILLS, AND ABILITIES:
- Knowledge of the principles, methods, techniques, and practices of social work.
- Skill in interviewing and assessing treatment needs of people served.
- Ability to communicate effectively both written and oral.
- Good organizational skills and the ability to plan and implement consumer services.
- May require abilities to supervise the work of others; to interpret programming methods and procedures; and to provide guidance to other staff.
- Ability to work under conditions requiring physical stamina and the ability to work in a stressful environment.
