GENERAL DESCRIPTION OF DUTIES AND RESPONSIBILITIES:


The Community Liaison will promote awareness of services available to people diagnosed with Intellectual and Development Disabilities (IDD) experiencing a mental health event.  This position will promote awareness of services available through Camino Real to people diagnosed with IDD.  Responsibilities include, but are not limited to assessing for crisis, develop crisis respite service plans describing the supports needed, collaborate with the individual’s service planning team on follow-up and relapse prevention activities (e.g., assisting with transition from crisis respite, addressing concerns/issues identified during monitoring visits, etc.).  Provides education to Camino Real staff and the community about the manner in which to engage individuals with IDD and their unique needs.

Monitor crises respite services for individuals diagnosed with ID in accordance with Camino Real Community Services policies and procedures.  Complete monthly skills training requirements for individuals with both an intellectual disability and a mental health diagnosis.  The job calls for self-motivated and energetic person to conduct observations and assessments relevant to the program on recommendations and intervention so as to increase the probability of positive outcomes for the individual in crisis and their families (e.g., systematic evaluation of team members, varying site consultations, functional assessment and analysis, internal/external interviews, provide MCOT teams, law enforcement and internal collateral.  This position will also be responsible for monitoring of crisis respite plan strategies, monitoring program outcomes, developing, and training on tracking forms, etc.).  This position will be situated at the Zavala Work Center in Crystal City, Texas and will require some travel within Dimmit/Zavala Counties.  The position will be under the supervision of the Crisis Intervention & Eligibility Determination Specialist.

MINIMUM JOB QUALIFICATIONS:

Applicants must meet the following minimum qualifications to be eligible for the position:
 
  • Bachelor’s Degree from an accredited college or university, with a major in (or the equivalent to a major, i.e., 24 hours of course work) in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, education psychology, early childhood education, or early childhood intervention; OR a registered nurse.
  • Must meet TAC Qualified Mental Health Professional (QMHP) qualifications.
  • You must possess good computer skills.
  • Has at least two (2) years of experience working directly with persons diagnosed with intellectual or other developmental disabilities (IDD) who have mental health and behavior support needs or linking people with IDD to mental health supports, in addition to one year of required experience.

PREFERRED JOB QUALIFICATIONS:
 
  • At least two (2) years of experience working with individuals diagnosed with IDD.
  • Lives in the service area.
  • English/Spanish skills; demonstrated ability to read, write, and speak English and Spanish.

KNOWLEDGE, SKILLS, AND ABILITIES:
 
  • Ability to complete documentation as required and reports accurately and timely.
  • Ability to complete the required training.
  • Skilled in customer relations with strong professional communication skills.
  • Works well with co-workers and teams.
  • Possess good computer skills.
  • Knowledge of local, state, and federal laws and regulations relevant to program.
  • Ability to submit required documentation and/or reposts accurately and timely.
  • Highly motivated, excellent work ethic, high degree of quality work.
  • Ability to work under conditions requiring physical stamina and the ability to work in a stressful environment.
