Camino Real Community Services is a Trauma-Informed Care Agency and implements the principles of Trauma-Informed Care.

GENERAL DESCRIPTION OF DUTIES AND RESPONSIBILITIES:
 
The Crisis Care Coordinator provides compassionate, trauma-informed support to individuals experiencing emotional or personal crisis.  This role focuses on stabilizing patients during mental health emergencies and coordinating follow-up care.  Key responsibilities include  conduct comprehensive screening and risk assessments for suicidal behavior, mental illness, substance use, and other vulnerabilities at intake.  Monitor and report on patients’ physical, mental, and emotional well-being to the treatment team.  Provide solution-focused, strength-based, person-centered interventions to de-escalate crisis.  Identify and connect individuals to additional clinical services and support networks.  Perform or consult on follow-up contacts with individuals and families.  Develop and implement patient treatment plans and comprehensive discharge plans, including relapse prevention strategies.  Ensure a safe and healthy environment for staff and clients, documenting and communicating client activities.  Occasionally drive a company vehicle to transport clients to scheduled activities.  Expertise in crisis response using evidence-based best practices.  Ability to multitask, handle on-site crises, and adapt to changing needs with compassionate care and attention to detail.  This position reports to the Director of Crisis Services.

MINIMUM JOB QUALIFICATIONS:
 
  • Bachelor’s degree from an accredited college or university with a major (or the equivalent to a major) in social, behavioral, or human services field with at least 24 hours of college credit in behavior/social science (psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education psychology, and early childhood intervention.  Must meet Qualified Mental Health Professional (QMHP) requirements.
  • Three (3) years of full-time work experience in human services case management.
  • Demonstrate ability to read, write and speak English and Spanish.

PREFERRED JOB QUALIFICATIONS:
 
  • Graduate Degree in counseling or social work.
  • Prior experience at a behavioral health or community mental health center setting.
  • Prior knowledge of the principles, methods, techniques and practices of social work and chemical dependency, the functioning of mental health hospitals and/or out-patient mental health clinics and schools.
  • Prior knowledge and experience with trauma-informed clinical interventions for mental and emotional wellbeing.

KNOWLEDGE, SKILLS, AND ABILITIES:
  • Knowledge of individual needs/characteristics associated with severe and persistent mental illness.
  • Knowledge of community-based service delivery system and the role of case management.
  • Knowledge of community resources, how to access those services and use them effectively.
  • Knowledge of the principles, methods, techniques and practices of psychology, behavioral science, social work, chemical dependency, and the functioning of mental health hospitals and clinics.
  • Skills with needs assessments, treatment planning and responding to consumer’s changing needs.
  • Skill with contributing to a treatment team of physicians, nurses, counselors, and others.
  • Skills for independent, organized, and efficient work and effective verbal and written communication.
  • Skill for documenting work done as well as drafting reports and recommendation for administrative use.
  • Ability to work cooperatively and effectively with other professionals, individuals, and groups.
  • Ability to exercise tact, show initiative, and use good judgment in dealing with people even in crisis.
  • Ability to develop productive and outcome focused relationships with individuals and family members.
  • Ability to recognize and overcome interagency obstacles for accessing services for individuals.
  • Ability to identify problems/gaps in service.
  • Ability to advocate effectively for an individual in the target population.
