GENERAL DESCRIPTION OF DUTIES AND RESPONSIBILITIES:


The Service Coordinator (SC) is responsible for handling Referrals for Intake services, coordinating their overall programs, documenting, and facilitating and monitoring the Person Directed Plan (PDP) and coordinating and linking services as identified by the PDP.  The Service Coordinator contacts individuals and their families to gather social and medical history, requests documentation from school districts, doctors, or other agencies to determine the onset of Intellectual Developmental Disability (IDD) prior to the age of eighteen (18) years old and completes a Social/Medical History Intake.  The Service Coordinator also discusses services for General Revenue (GR), Texas Home Living (TxHmL), Home Community Services (HCS) and places the individual on the HCS Interest List.  The Service Coordinator provides Service Planning, Monitoring, Assessment, and Crisis Prevention services to individuals on a caseload not to exceed forty-five (45) individuals.  The Service Coordinator will assist individuals and families in seeking community placement as requested, conduct Permanency Planning Reviews, enroll individuals in the Community First Choice (CFC) program, and assist with planning for admissions and discharges to and from the State Supported Living Centers (SSLC) and State Hospitals.  The Service Coordinator will meet productivity standards and appropriately maximize revenue by ensuring all service provision activities are documented and correctly submitted within the agency timeframes.  This position will primarily serve individuals residing in Karnes, Wilson, and Atascosa counties.  This position will be housed at the Wilson Admin office in Floresville, Texas.  This position is also required to be trained in trauma-informed care.  The Service Coordinator is under the direction of the Access & Intake Director.

MINIMUM JOB QUALIFICATIONS:

Applicants must meet the following minimum qualifications to be eligible for the position:
 
  • Bachelor’s Degree from an accredited college or university, with a major in (or the equivalent to a major), in social, behavioral health or human service field.

PREFERRED JOB QUALIFICATIONS:
 
  • Prior experience as a Service Coordinator/Intake Worker/Case Worker/QIDDP field with the IDD population.
  • Spanish/English skills (bilingual), demonstrate ability to read, write and speak Spanish.
  • Lives within the service area.

KNOWLEDGE, SKILLS, AND ABILITIES:
 
  • Knowledge of the principles, methods, techniques, and practices of service coordination.
  • Knowledge of community resources.
  • Ability to coordinate, plan and develop services as part of a team and individually.
  • Ability to develop productive relationships with individuals, their family members that focus on the individual’s specific goals and dreams.
  • Ability to advocate effectively for the best interest of the individuals served.
  • Ability to work cooperatively and effectively with other professionals, service planning team members and other agencies.
  • Ability to work independently, organize work efficiently and communicate effectively.
  • Ability to meet stringent timeframes for submission of paperwork.
  • Ability to exercise tact, initiative, and good judgement in different situations.
  • Ability to complete documentation of contacts for billing purposes within 48 hours of the contact.
  • Must be able to complete all required contacts for assigned individuals on a monthly basis.
