GENERAL DESCRIPTION OF DUTIES AND RESPONSIBILITIES:

The PASRR service coordinator position is responsible for service planning, monitoring, assessing and identifying needs and goals of consumers diagnosed with intellectual and developmental disabilities relating to personal outcomes, including recommendations for interventions and support services. Arranges and collaborates with service providers and nursing facilities in order to link consumers with outcome-focused services. The service coordinator will be responsible for managing a caseload not to exceed forty (40) persons. The caseload will consist of consumers who reside in a nursing facility or are at risk of being admitted into a nursing facility and utilize the Preadmission Screening and Resident Review (PASRR) funding stream. The service coordinator is responsible for making contacts with assigned consumers in accordance with their service coordinator assessment. Service coordinators are responsible for completing annual and quarterly Individual Service Plans (ISPs) for each PASRR person on their caseload and, completing timely and accurate documentation of the delivery of service coordinator services. Requires flexibility in scheduling and significant travel within the service area. The job calls for a self-motivated and energetic person to assess consumerís outcomes and needs and provide monitoring of services to consumers living in nursing homes. The caseload will be primarily concentrated in Atascosa, Frio, LaSalle and Wilson counties. Service coordinators are required to meet set productivity standards on a monthly basis. This position will be housed at the Lytle Headquarters Office in Lytle, Texas and will be directly supervised by the Service Coordinator Director.

MINIMUM JOB QUALIFICATIONS:
  • Bachelorís degree from an accredited college or university with a major in social, behavioral, or human services.
  • Must possess proficient computer skills.
PREFERRED JOB QUALIFICATIONS:
  • Experience as Service Coordinator/case worker field with the IDD population.
  • Spanish/English skills; demonstrate ability to read, write and speak Spanish.
KNOWLEDGE, SKILLS AND ABILITIES:
  • Knowledge of the principles, methods, techniques, and practices of service coordination.
  • Knowledge of community resources.
  • Ability to coordinate, plan and develop services as part of a team and individually.
  • Ability to develop productive relationships with individualís, their family members that focus on the individuals specific goals and dreams.
  • Ability to advocate effectively for the best interest of the individuals served.
  • Ability to work cooperatively and effectively with other professionals, team members and other agencies.
  • Ability to work independently, organize work efficiently and communicate effectively.
  • Ability to meet stringent timeframes for submission of paperwork.
  • Ability to exercise tact, initiative and good judgment in different situations.
  • Ability to complete documentation of contacts for billing purposes within 48 hours of the contact.
  • Must be able to complete all required contacts for assigned consumers on a monthly basis.
  • Ability to work under conditions requiring physical stamina and ability to work in a stressful environment.
