Agency vehicle may be available; however, mileage is reimbursed for some activities requiring use of personal vehicles. Selected applicant must be able to attend approximately 5 working days of staff development orientation in Lytle, TX.

GENERAL DESCRIPTION OF DUTIES AND RESPONSIBILITIES:

The Service Coordinator position is responsible for service planning, monitoring, assessing and identifying needs and goals of consumers diagnosed with intellectual and developmental disabilities relating to personal outcomes, including recommendations for interventions and support services. Arranges and collaborates with service providers and community agencies in order to link consumers with outcome-focused services. The service coordinator will be responsible for managing a caseload not to exceed forty-five (45) persons. The caseload will consist of consumers who utilize different funding streams to include, GR, HCS, and TXHML. The Service Coordinator is responsible for making contacts with assigned consumers in accordance with their level of need based on their service coordination assessment. Service coordinators are responsible for completing an annual person-directed plan (PDP) for each person on their caseload and, completing timely and accurate documentation of the delivery of service coordination services. Requires flexibility in scheduling and significant travel within the service area. The job calls for a self-motivated and energetic person to assess consumerís outcomes and needs and provide monitoring of services to consumers living in their natural homes, host home settings and/group homes. This position also requires excellent documentation and time management skills. The caseload will be concentrated primarily in Maverick County, however depending on agency need, this position might be assigned consumers residing in other surrounding counties. Service coordinators are required to meet set productivity standards on a monthly basis. . This position will be housed at the Maverick IDD Office in Eagle Pass, TX., and will be directly supervised by the IDD Service Coordination Supervisor for that area.

MINIMUM JOB QUALIFICATIONS:
  • Bachelorís degree from an accredited college or university with a major in social, behavioral, or human services.
  • Must possess proficient computer skills.
  • Application must be Bi-lingual in English and Spanish
PREFERRED JOB QUALIFICATIONS:
  • Prior work experience as a Service Coordinator or Case Manager working with people with intellectual or developmental disabilities.
  • Must live within the service area.
KNOWLEDGE, SKILLS AND ABILITIES:
  • Knowledge of community resources.
  • Must possess excellent verbal and written skills.
  • Ability to coordinate, plan and develop services as part of a team and individually.
  • Ability to develop productive relationships with individuals and their family members that focuses on the individualís specific goals and dreams.
  • Ability to advocate effectively for the best interest of the individuals served.
  • Ability to work cooperatively and effectively with other professionals, team members and agencies.
  • Ability to work independently, organize work efficiently and communicate effectively.
  • Ability to meet stringent timeframes for submission of paperwork.
  • Ability to exercise tact, initiative and good judgment in different situations.
  • Ability to complete documentation of contacts for billing purposes within 48 hours of the contact.
  • Must be able to complete all required contacts for assigned consumers on a monthly basis.
